Start with who's actually involved
Depending on the situation, a home care team can include a primary doctor, a hospice or home health nurse, aides, a social worker, and a chaplain. Not every situation involves all of these roles, and that is normal.
What's worth writing down for each contact
A useful contact sheet has more than a name and a number.
- Name and role — what they specifically help with
- Phone number, and the best way to reach them (call or text)
- Typical hours or on-call coverage
- Anything specific to bring up with them versus someone else on the team
Keep a separate line for after-hours
Many care teams use a different number for nights, weekends, or urgent situations. Write it down before you need it, not while you're searching for it at 2am.
Update it when the team changes
Contact sheets go stale the same way binders do. When a provider changes or a new person joins the team, treat updating the sheet as part of that change, not a separate task for later.
