What belongs in a care binder

A useful care binder does not need to be perfect. It needs current pages that helpers can find quickly: contacts, handoffs, medication lists, household notes, visit questions, schedules, and document locations.

These pages are designed to be filled out in your browser, printed, and kept offline. Leave anything blank that does not apply.

How to print and use these pages

  • Start with the page people ask for most often, usually contacts, medication list, or shift handoff.
  • Print a fresh copy when important details change, and remove the old one from the binder.
  • Keep sensitive papers in a safe place, and use locator sheets to say where they are kept.
  • Follow instructions from the care team for medical questions, medication changes, and urgent concerns.
